Our client, Hospital Universitario Ramón y Cajal, is a large public hospital based in Madrid. This hospital serves as a reference center for more than 600,000 people. With over 900 beds, the hospital is renowned for its extensive healthcare services and its continuous commitment to integrating technological advancements into patient care.
The Challenge
The hospital faced a major challenge transitioning to a new Regional Healthcare Information System (HIS) called HCIS. The integration required synchronizing existing departmental solutions with the HCIS through files and databases. A key part of this was converting data into the HL7 format, ensuring seamless communication across various departments, including Emergency, Inpatient, Day Hospital, and Outpatient Clinics.
The project included a broad range of HL7 v2 events critical for patient location monitoring and census management. The integration required handling a comprehensive list of HL7 events, such as:
- Patient creation (
ADT^A28). - Patient data modification (
ADT^A31). - Medical record merging (
ADT^A40). - Admission and cancellation (
ADT^A01andADT^A11). - Transfer and cancellation (
ADT^A02andADT^A12). - Discharge and cancellation (
ADT^A03andADT^A13). - Visit modification (
ADT^A08). - Bed exchange (
ADT^A17). - Episode transfer (with another patient) (
ADT^A45).
Another significant challenge was merging demographic and healthcare activity data with the previous HIS. This was essential to maintain key functionalities, especially in inpatient and emergency departments. Additionally, incorporating sensitive information like patients’ food allergy data and other vital records into the system was crucial.
The Solution
To address these challenges, we implemented a comprehensive and multi-faceted solution. Our approach began with detailed interoperability consulting and the development of a customized training program for hospital staff to facilitate the integration process and familiarize them with the new HIS.
The solution relied on deploying our advanced integration engine, Meditecs Smart Connect, designed to handle high volumes of messaging without overloading the source system—a common challenge. The solution also included a robust mechanism for controlling message processing, ensuring data synchronization across the hospital’s databases in the correct order. This guaranteed data integrity, a complex task given the system’s scale.
We also developed a comprehensive error catalog to streamline troubleshooting, improving the system’s resilience and reliability. Additionally, we updated internal processes to align with the hospital’s databases and introduced advanced automated validation tools.
To ensure the long-term success of the integration, we established a monitoring and preventive maintenance service, supported by a comprehensive incident management system. This strategy has proven effective, with the system currently processing around 300,000 HL7 messages monthly, serving over 400,000 patients.
Follow up
Years later, this solution continues to enhance the hospital’s operations, enabling seamless integration between the new HIS and the hospital’s existing departmental solutions. The volume of messaging handled by the system has steadily increased since the project’s deployment, reflecting the growing demands of the hospital and the robustness of the integration solution.
